The gap between finishing rehab and being safe at home is where progress gets lost. We bridge it with rides, meals, reminders and company, so your patient keeps the ground they gained.
Physician offices
You cannot see what happens between appointments. A note from us about how someone is eating, moving and managing their medications is information your nurse line does not otherwise get.
Case managers & social workers
When the family lives out of state, someone local needs to lay eyes on the situation. Send us the name and we will follow up with the family promptly.
